Once you have collected information from your form, open the response spreadsheet. Go to the add-ons tab, select "get add-ons", and search for "Save as Doc". Once the add-on is installed, select it from the drop down menu and click start. A box will pop up on the right side of the spreadsheet. Click on "select all data", title the document, click on "page break after each row" if desired. Lastly, click on "save as doc".
